Procedure of Changing of Name


Any registered consumer of the Kabankalan City Water District with active water service connection.

Monday to Friday 8:00 a.m.- 4:30 p.m.

No noon break

Charges - NONE

Procedure:

1. Go to the customer Service Clerk and ask for requirements for Change of Name.


2. Fill-up the Memorandum of Agreement, have it signed and hand it to the Customer Service clerk along with the deed of sale or any proof of transfer.
